By submitting this form, I hereby authorize the staff of Club Nine Baseball to act on my behalf according to their best judgment in the event of any emergency requiring medical attention. I hereby waive and release Club Nine Baseball from any and all liability for any injury or illnesses occurring while my child is receiving instruction.
